Terms and Limitations

Cannot be combined with dental insurance.
Only good for use at our office. If referred to specialist, they will not honor this discount.
Discount cannot be used on dental treatment needed due to any type of injury where a lawsuit and therefore outside medical, care, disability or workman’s comp type insurances are involved.
This plan is NON-transferable. Family members or friends may not use your membership benefits.
The member is responsible for scheduling and maintaining appointments within the 12-month period.
This plan in NON-refundable. No refunds will be given if member does not use dental plan.
Rates are subject to change annually.
Payment is due at time of service. If CareCredit is used, discount will be reduced to 10% due to merchant fees.
This offer cannot be combined with any other offers.
Missed appointments without a 48-hour notice will result in forfeiture of your discount.
For orthodontic treatment, participant must remain a plan participant the entire duration of orthodontic treatment or forfeit the $500 discount.
Dental services only, products are not included.
Cosmetic Injectables not included.
